مقایسه‌ی شاخص‌های عملکردی بخش اورژانس قبل و بعد از اجرای برنامه‌ی تحول سلامت در بیمارستانی آموزشی در شهر زابل

چکیده

مقدمه: طرح تحول نظام سلامت ایران، در اردیبهشت 1393 با اهداف عدالت در دسترسی به خدمات سلامت، ارتقای کیفیت خدمات و حفاظت مالی از بیماران در برابر هزینه‌های بالای خدمات بیمارستانی در بیمارستان‌های دانشگاهی اجرا شد. هدف این پژوهش، مقایسه‌ی شاخص‌های عملکردی بخش اورژانس قبل و بعد از اجرای طرح تحول نظام سلامت در بیمارستانی آموزشی در شهر زابل بود.


شیوه‌ی مطالعه: این مطالعه‌ی توصیفی- تحلیلی در سال ۱۳۹۷ در بیمارستانی آموزشی وابسته به دانشگاه علوم پزشکی زابل انجام شد. اطلاعات شاخص‌های عملکردی بخش اورژانس دو سال قبل و دو سال بعد از اجرای طرح تحول نظام سلامت به روش سرشماری انتخاب شدند. داده‌های پژوهش با استفاده از نرم‌افزار SPSS نسخه‌ی ۱۹ و با آزمون آماری تی تحلیل شدند.


یافته‌ها: نتایج نشان داد که اجرای طرح تحول نظام سلامت بر درصد بیماران تعیین تکلیف شده‌ی کمتر از 6 ساعت مؤثر بوده است که از نظر آماری معنی‌دار بود (0/004 = p value). با این وجود، اجرای طرح تحول نظام سلامت، تأثیر معنی‌داری بر درصد بیماران تعیین تکلیف شده‌ی کمتر از 12 ساعت، درصد احیای موفق، درصد احیای ناموفق و درصد ترک با مسؤولیت شخصی نداشته است (0/05 < p value).


نتیجه‌گیری: اجرای طرح تحول نظام سلامت باعث بهبود شاخص درصد بیماران تعیین تکلیف شده‌ی کمتر از 6 ساعت شده است. با این وجود، اجرای این طرح بر بیشتر شاخص‌های عملکردی بخش اورژانس بیمارستان مورد مطالعه مؤثر نبوده است. بنابراین، ضروری است که زیرساخت‌های لازم از جمله تأمین منابع مالی و انسانی کافی، برگزاری دوره‌های آموزشی اثربخش در زمینه‌ی نحوه اجرا و پایش دستورالعمل‌های طرح تحول نظام سلامت و اهداف حاصل از آن فراهم گردد.
