شمارش واحدهای کربوهیدرات در مبتلایان به دیابت نوع یک و اثر آن در کنترل وضعیت متابولیک: مقالهی مروری

چکیده
مقدمه: با توجه به پاتوژنز بیماری دیابت و از طرف دیگر ضرورت استمرار درمان با استفاده از تزریق انسولین، به کارگیری روشهایی منطبق با سبک زندگی روزانه، مهمترین گام برای کنترل مؤثر همراه با حفظ سلامت روحی و جسمی و تشویق افراد به پیگیری درمان میباشد. مطالعهی حاضر با هدف بررسی تأثیر شمارش واحدهای کربوهیدرات در مبتلایان به دیابت نوع یک انجام شده است.
شیوهی مطالعه: پژوهش حاضر یک مطالعهی مروری است. جستجو با استفاده از کلید واژهها از جمله دیابت نوع یک، دیابت نوع دو، انسولین و شمارش واحدهای کربوهیدرات به زبان فارسی و انگلیسی در پایگاههای معتبر از جمله PubMed، Wiley، Elsevier، Science Direct و SID از سال 2000 تا 2019 انجام شد و مقالات مورد بررسی قرار گرفتند.
نتیجهگیری: یافتهها نشان داد، جهت به کارگیری شمارش واحدهای کربوهیدرات، دو مفهوم باید در نظر گرفته شود. در گام نخست فاکتور حساسیت انسولین (قانون 1800 و قانون 1500) و در گام دوم، مفهوم دیگری به نام نسبت انسولین به کربوهیدرات (قانون 500، 450 و در کودکان قانون 300). در پایان میزان کل انسولین هدف، از مجموع انسولین مورد نیاز برای اصلاح گلوکز خون پیش از غذا و وعدهی غذایی به دست میآید. تنظیم میزان انسولین بر اساس میزان دریافت کربوهیدرات به کنترل بهتر وضعیت گلیسمیک، انعطافپذیری الگوی تغذیهای و بالا بردن کیفیت زندگی مؤثر است.
